Patient Forms
If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Dental Payment Plans
We accept patients with and without dental insurance. Payment is due at the time service is rendered unless other arrangements have been made in advance.
Here at Super Dental Family Center, we offer personalized payment plans to ensure that your dental needs are met in an affordable manor. We offer interest free financing options with Care Credit which can be used for most out of pocket expenses.
Payment Options
For your convenience, we accept cash, personal checks, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
Dental Insurance
We accept most dental insurance including employer sponsored or individual "self insured" plans. Please do not hesitate to contact us directly to find out if we accept your specific dental plan.
We will estimate your portion and ask you to pay that amount at the time of service. Please be advised, this is only an ESTIMATE and does not guarantee that your insurance company will cover the remainder of the fee. The insurance company makes the final determination as to how much they pay. We can only offer an estimate. The patient is responsible for payment of any fees their insurance company does not cover.
Our Insurance Coordinator will process all insurance claims on your behalf and work with your insurance to ensure you receive the maximum benefit allowed.
